Adding Cc: Bcc: ?

I've tried a number of different ways to add a Bcc: and none do the trick. Anyone had any luck adding Cc: or Bcc: to this form? I'm assuming whatever the additional code is, it goes in here --> mail("name@company.com", ...

Any help is appreciated
